Join us for a two-part program on Advance Directives. Ellen Taylor, Dementia Care Specialist at the Aging & Disability Resource Center of Dane County, will lead a conversation about how to communicate what matters most to you and ensure your wishes are known in your health care. You'll learn about advance care planning options and the steps involved in completing them. The second hour will be a hands-on workshop where you can ask questions and complete Wisconsin state forms.
